2008 Swim Lesson Request Form

Description: The BCC Sharks coaching staff provides private swim lessons for a reasonable fee to any Shark swimmers that are interested in getting extra help outside of the regular team practice times. The BCC Sharks Swim Team is not involved in managing nor receives any proceeds from the swim lesson program. As a benefit to our coaching staff, Balcones Country Club does allow their pools to be used for the private swim lessons. Any arrangements for private swim lessons is strictly between the coaching staff and families.

Instructions: Fill out all the information requested below. Be sure and double check that you have typed your e-mail and contact phone numbers correctly. This is the only way the coach will be able to contact you to confirm your swim lesson request. By submitting this form, you have not scheduled the swim lesson yet. You have submitted a request, which must be confirmed by a coach either by e-mail or phone call before the lesson is scheduled. If you have not had a response back within 24 hours, please contact Coach Nurrie Wilson.

Note: After clicking the Submit button (only click once), you should receive a "Swim Lesson Request Summary " page which will serve as your confirmation that your request has been submitted.
